Chronic disease linkages

Many older people live with multiple complex chronic conditions, requiring a range of health and community service interventions.  Working with BDHP networks and the Chronic Disease Theme, our chronic disease and end of life stream focuses on improving the evidence base to promote optimal wellbeing for older people living with chronic conditions and their family and carers. It also focuses on promoting quality of life for older people toward the end of their life. Research in this theme involves surveying existing relevant initiatives, as well as the development and testing of innovative clinical, behavioural and health service interventions across a range of health care settings to enable integrated and person and family centred care models. The theme incorporates a range of knowledge translation approaches to facilitate optimal outcomes for older people and their family and cares across the trajectory of chronic diseases.
